Our Practice Nurses are specially trained in travel health and can offer the full range of travel immunisations and advice, wherever in the world you may be going.

Once completed, please drop the form in to us at the surgery or email it to us at firstname.lastname@example.org. Once you have returned the completed form please contact reception and we can book you an appointment with the Practice Nurse.
It is best to see the Nurse about six to eight weeks before you travel. If you need to have a course of vaccinations, these can be at different times over a number of weeks. The time between injections will allow your body to respond to the vaccine, so you develop immunity ready for when you arrive at your destination. If you have less than six to eight weeks before your planned travel date please still contact us as it may still be possible to get vaccinated.
Please be aware that only some travel vaccinations are available free on the NHS and you will need to pay for most vaccinations.
